Ovarian Cysts
Fluid-filled sacs on the ovary. Most are harmless and resolve on their own; some need treatment.
Symptoms
- Pelvic pain or dull ache on one side
- Bloating or fullness
- Pain during intercourse
- Irregular periods
- Sudden severe pain (possible rupture or torsion — an emergency)
Causes
- Functional cysts from the normal ovulation cycle
- Endometriomas ("chocolate cysts") due to endometriosis
- Dermoid cysts and cystadenomas
- Polycystic ovaries
Risk Factors
- Hormonal fertility medicines
- Endometriosis
- Pregnancy
- Previous ovarian cysts
- Severe pelvic infection
Diagnosis
- Pelvic ultrasound
- Tumour markers (e.g. CA-125) when indicated
- Hormone tests
- Repeat scan to check whether the cyst resolves
Treatment
- Observation with a repeat scan for simple cysts
- Hormonal medicines to prevent new functional cysts
- Laparoscopic cystectomy — removes the cyst while preserving the ovary
- Emergency surgery for torsion or rupture
Recovery
- Laparoscopic cystectomy: usually 1 day in hospital
- Most women return to normal activity within 1–2 weeks
- Follow-up ultrasound as advised
